How Much Do Medicare Agents Make Per Client in 2027?

At a glance

For 2027, the national Medicare Advantage maximum compensation is $725 for an eligible initial enrollment and $363 for a renewal. Standalone Part D maximums are $130 initially and $65 for renewals. These are compensation ceilings—not guaranteed payments on every application.

I located the June 1, 2026 CMS memorandum, hosted by a brokerage, confirming the following amounts. psmbrokerage.com

Plan / market Initial compensation maximum Renewal maximum
Medicare Advantage — national rate, including Texas $725 $363
Medicare Advantage — Connecticut, Pennsylvania, Washington, DC $816 $408
Medicare Advantage — California and New Jersey $902 $451
Medicare Advantage — Puerto Rico and U.S. Virgin Islands $495 $248
Standalone Part D — nationwide $130 $65

Does every new client generate an initial commission?

No. New to your agency does not necessarily mean an initial-compensation enrollment. A beneficiary moving between like plan types generally falls under renewal compensation rules. CMS distinguishes initial enrollments and unlike-plan-type changes from renewals and like-plan-type changes. www.cms.gov

Before projecting income, verify:

  • Whether the specific plan pays a commission.
  • Whether the enrollment qualifies for initial or renewal compensation.
  • Your contract and any agency split.
  • Payment timing, proration, and potential chargebacks.

CMS’s training guidance includes compensation proration and recoupment requirements. cms.gov

Medicare Supplement compensation is separate; do not apply the Medicare Advantage table to Medigap policies.

The message for agents: Understand the difference between a commission maximum, an actual payment, and business profit. Build your income plan around accurate contracts and client retention—not the assumption that every application pays $725.
